Six Senses Douro Valley: the full spa option
Six Senses near Lamego has a large spa and fitness area, indoor and outdoor pools, treatment rooms and several places to eat. Choose it when spa time and the hotel are a main part of the trip.
It is not within walking distance of Pinhão. Arrange road transport or use the hotel's transfer options, and leave enough time to use the facilities.

Quinta da Pacheca: wine hotel with a vineyard spa
Quinta da Pacheca near Régua has rooms on a wine estate, wine visits, a restaurant, a swimming pool and the Vineyard Spa. The official hotel page lists a hydrotherapy circuit and treatments using grapes and other natural products.
It is a good choice if you want both wine and spa time, but do not need a full spa holiday. A car or booked transfer is still useful for dinner or visits away from the property.
Quinta do Vallado: wine hotel with spa facilities
Quinta do Vallado near Régua is a wine hotel with a pool, winery visits, a jacuzzi, Turkish bath, sauna and bookable treatments. Choose it if you want to stay on a wine estate but do not need a large resort.
Check the rules for spa access and book treatments in advance. Use a car, taxi or arranged transfer because the hotel is not an easy walk from central Régua.
Quinta Nova: a wine stay without a full spa
Quinta Nova Winery House focuses on the estate, wine, Terraçu's restaurant and a small pool rather than a full spa. Its official site also lists three marked walks on the estate and a check-in and check-out transfer from Ferrão station or pier for guests.
Choose it if wine is the main reason for your stay, and arrange meals and transport with the property. Do not expect the facilities of a full spa resort.

Location and the need for a car
Properties near Régua are easier to combine with a road itinerary, while remote estates may depend on a station pickup, taxi or hotel-arranged transfer. Straight-line map distance is a poor guide on hillside roads.
Before booking, ask how you will reach the hotel, whether dinner operates on your dates, how spa treatments are reserved and whether pools are indoor, outdoor, heated or seasonal. Do not assume every facility is available to every room type or at all times.
